We don't ask for a name or a reference number. That seems less secure but isn't: the email only goes to the address you type in here, and anyone who can read that mailbox already has your request anyway. Adding a name would mainly mean you get "not found" because you entered "J. de Vries" and now type "Jan de Vries", while your request is there all along.
Then there is no open request from the past year for this address. Check whether you used a different address, and have a look in your spam folder too.
